We specialize in Short Term Care, Long Term Care, Rehabilitation, Sub-acute Care, and Alzheimer's Specialty Care. Our facility also hosts an Adult Day Health Center, and Senior Transportation.

Blaire House of Tewksbury is seeking an energetic Social Worker to uphold our high standards for senior care!
Job Summary:
- Responsible for ensuring the rights and dignity of residents.
- Recommend, develop and document discharge plan.
- Facilitates the Behavioral Committee weekly according to facility policy.
- Implementing social service plan and for periodically reviewing plan in conjunction with overall plan according to established policy.
- Providing social intervention, such as counseling, as patient advocate, as liaison with community, or regarding financial or emotional issues.
- Maintaining social service progress notes on patient medical record which reflect any changes in patient psycho-social or physical status, evaluation of family interrelations, identification of new problem areas, and review of social service plans, goals, and progress made
- Possess genuine interest in geriatric population.

Qualifications:
- EDUCATION: B.A. degree in Human Services or related field.
- TRAINING AND EXPERIENCE: Must have a genuine interest in working with geriatric patients and a sincere concern for their welfare and happiness.
